Who are the Community Mental Health Services Floating Support Team?

The Floating Support Team was set up as part of the council’s Supporting People programme. The Floating Support Team provides support for adults aged 18+ who may need help to enable them to have a greater independence within their own tenancies. This support is defined as ‘housing related’ and it covers a wide range of areas of need within a tenancy / community-focused remit.

We aim to enable, remind or assist individuals to maintain their accommodation, and maximise their potential in a person-centred way. Individuals are supported to achieve their aims and goals and empowered to strive to meet their aspirations as much as possible. Each individual will receive allocated hours (up to 10 hours per week) for a maximum of two years. Individuals are reviewed regularly and also signposted to other agencies or services identified to support them to become independent.

Who is eligible and how do people access the service?

Currently, individuals must have a primary diagnosis of mental health-related problems; this can include dual diagnosis. The referral pathway is via Contact Cares on 01744 676767. The manager will arrange an assessment visit to you with staff from the team. Following assessment and discussion, a support worker will be allocated along with agreed days and time of visits etc.
