Choose to Change, delivered by ABL Health, is a specialist weight management service for adults aged over 18 years and with a Body Mass Index over 40 (or over 35 with certain health conditions). The programme helps individuals to make small, realistic and effective changes to their lifestyle to support weight loss and health improvements.
The programme offers support for up to 12 months, encouraging lifelong change rather than short term traditional diets. It helps people to break down their barriers to sustained weight loss, become more active and make positive food choices.
Starting with an assessment, which includes a discussion about personal barriers, eating habits, challenges and goals, the majority of people will start on a friendly and supportive group. 1:1 support is available where a group environment is not suitable and the service also offers individual or group specialist psychological therapy for people with complex eating habits and issues around emotional eating.
Referral is via the Healthy Living Team. Self-referrals or health professional referrals are accepted. For more information about the programme please contact the Choose to Change team on 01744 778 359, email us or visit the Choose to Change website.
